The warm and confident manner of our team members is key to the well-being of our residents.

 

The Lev at San Antonio begins with a rigorous screening and application process. We select only the most proficient and motivated candidates to join our team, from nurses’ aides to therapists. Once on board, our team members are continuously trained in current treatment modalities to keep them up-to-date. They also receive extensive sensitivity training, which is reflected in their interactions with residents, and ultimately results in improved resident outcomes.

Administrator

Krystal Carmona, LNFA

Krystal Carmona is a Licensed Nursing Facility Administrator who oversees the daily operations of the community. Krystal has worked in long-term care for seventeen years, advancing from her previous roles as Medical Records, Administrative Assistant and Director of Human Resources. She attended St. Edward’s University for both undergraduate and graduate studies and holds a master’s degree in Organization Development. More than just an operations leader, she is an advocate for her residents and team members; she believes in supporting a safe and enriching community environment.

Business Office Manager

Carolyn Nelson

Carolyn Nelson is the Business Office Manager at The Lev at San Antonio. She has over twenty-five years in the medical field with seven years of management experience. Carolyn is aCalifornia native and made the big move to Texas to be closer to family. She finds joy in helping others, especially working with families to make their loved one’s transition easier.

Director of Social Services

Kimberly Deike, LBSW

Kimberly Deike is the Director of Social Services at The Lev at San Antonio. Kimberly earned her undergraduate degree in Social Work and Psychology at Our Lady of The Lake University. She then completed advanced studies in Aging and Dementia from Texas State University. She is a Licensed Baccalaureate Social Worker, and a Family Violence Prevention Specialist. Kimberly has worked in the Senior Living and Skilled Nursing Industry for ten years. She is an advocate for Culture Change in Long-term Community Living and has been a Certified Eden Associate since 2015. Kimberly isnaturally approachable and provides a high level of support and coordination of services to residents and their families.

Director of Business Development

Stephanie Esqueda

Stephanie Esqueda is a San Antonio native and has ten years of experience in the medical field and six years in admissions and marketing. She has navigated the long-term care world as a Certified Nurse Aide, Restorative Aide, Medication Aide, and Admissions Coordinator. The knowledge and skills she obtained through providing direct patient care has carried over into her Director of Business Development role, as she is able to share her experiences with others as they transition to our home. Stephanie prioritizes building relationships with case managers, families, residents, and team members to bridge the connection between the community and each group.

Assistent Director of Nursing

Priscilla Arce, LVN

Priscilla Arce is a Licensed Vocational Nurse of seven years; she obtained her certification from a local career college and joined the company two years ago working her way up from Charge Nurse to Assistant Director of Nursing.  During her career, Priscilla has worked in a variety of healthcare settings, including hospitals, long-term care facilities and home health care homes. Priscilla believes in focusing on the whole person when caring for her residents. She is passionate about providing quality care while remaining mindful of everyone’s preferences and needs. In addition to her career in nursing, Priscilla enjoys spending quality time with her family and helping others in her community when she can.

Director of Nursing

Tiffany Guiterrez, RN

Tiffany Guiterrez, born and raised in this great city, is the community’s Director of Nursing and has many years of experience caring for the geriatric population. Her passion for nursing and drive to continue her career path from a Licensed Vocational Nurse of six years, led to accomplishing her professional goal of becoming a Registered Nurse. She attended Galen College of Nursing, and during that time held a managerial position in a fast-paced skilled nursing community. In this position, Tiffany discovered that she enjoyed being a mentor and leader and she would ultimately transition to the Director of Nursing for that home. Tiffany joined The Lev at San Antonio in December of 2023 and has helped nurture a clinical team full of compassion and dedication. In her spare time, she enjoys spending quality time with her family, friends and shopping!

Director of Human Resources

Desiree Arce

Desiree Arce is the Director of Human Resources. Desiree is currently enrolled in school to pursue a Bachelor of Science and Nursing degree with a minor in Administration. She has always held a passion for healthcare and has a professional goal to become a Registered Nurse so that she may continue being an advocate for others. Her hobbies include traveling, spending time outdoors, and taking dance classes. Desiree believes in having an open-door policy and strives to support each person surrounding her in any way she can.

Director of Rehabilitation Services

Mary Moreland, COTA

Mary Moreland is a Certified Occupational Therapist Assistant and the Rehabilitation Director at The Lev at San Antonio.  She has over fifteen years of clinical experience and seven years of management experience.  Mary leads an enthusiastic team of Clinicians who focus on the individual needs and capabilities of each of our residents.
